Applications to have a decision reviewed can be made either by or by filling out an application form which is available from the Tribunal's office or web site. The letter must include the applicants name, address and telephone number; the name of the Department or other Tribunal that made the decision; the date of the decision; a copy of the decision this is normally the letter sent to you by the Department or Tribunal; and brief reasons why you think the decision is wrong
