- Incorporating:, 1. Boandik Lodge Day Therapy Centre, , Occupational Therapy, Physiotherapy and Podiatry for people over 65 with assessed need. Support groups for people with Parkinson's disease or who have had a stroke. Ascot Club, Men's Group and twilight club provide recreational activities for older people.Transport is availble.
- 2. Home and Community Care Services, Includes domestic assistance, appointment support, shopping assistance, medication checks, personal care and social and recreational support. Out and About Community Program provides access to community social and recreational opportunities.
- 3. Community Aged Care Packages, Assistance available to enable frail, aged people to remain at home in the community. May include assistance with personal care, home help, assistance with medication and with maintenance of social lifestyle. Service can be offered over 7 days, with multiple visits available. Packages are tailored to individual's needs.
- 4. Extended Aged Care at Home and extended aged care at home domentia Assistance for frail, aged people with complex care needs, enabling them to stay at home in the community. Supported by Registered Nurse. May include assistance with persoanl care, home help, assistance with medication and maintenance of social lifestyle. Service offered over 7 days, with multiple daily visits available. Occupational Therapy, Physiotherapy and Podiatry services available.
- 5. Transition Care Programme, A short term transitional programme for older persons who have been in hospital and are expected to benefit from rehabilitation and support, in order for them to regain function and higher level of indepence.
