Eligibility
Older people living in parts of Burnside, Campbelltown, Norwood Payneham St Peters, Unley, Port Adelaide Enfield, Tea Tree Gully and Salisbury councils who need assistance to remain living at home. Assessment required by Aged Care Assessment Team for care packages. Therapy Services are for people aged 60 and over. Referral from GP may be required for various rehabilitative therapy services. Referral is not required for continence clinics

For services. Reduction in fees can be negotiated by application to the manager
Services
- Therapy services - speech pathology, podiatry, physiotherapy, occupational therapy, continence clinic, wound clinic and other rehabilitative support programs
- Assistance with Care & Housing for the Aged (ACHA) - assistance for financially disadvantaged older people who are living in insecure accommodation or at risk of homelessness
- Community Aged Care Packages
- Lifestyle Choices - fee for service program offering flexible support for older people living at home
- Talk Back Group - for people with speech difficulty
- Veterans Home Nursing
