Eligibility
People aged 60 and over, living in Marion, Holdfast, Mitcham and Unley council areas, who need assistance to remain living at home. Assessment for care packages required by Aged Care Assessment Team (ACAT). Referral from GP may be required for various rehabilitative therapy services and low level in-home support

For services. Reduction in fees can be negotiated by application to the manager

- Therapy services - podiatry, physiotherapy, occupational therapy, speech pathology, Parkinson's Disease Rehabilitation Clinic, rehabilitative and self-management support programs, chronic disease self-management, falls prevention, memory disorders, continence clinic, social work
- In-Home Care: assistance to help people remain independent and living in the community. Includes low level assistance, packaged care (multiple types of assistance) and nursing care
- Transition Care Program - short term help for older people who have been in hospital
