Eligibility
Frail aged and younger people with disabilities and their carers who live in the Barossa, Gawler, Lower North, Yorke Peninsula, Copper Coast and Wakefield Plains and are at risk of premature or inappropriate admission to residential care, and who meet program entry criteria. ACAT assessment required for Community Aged Care packages
