Aged care assessment teams (ACATs) are located throughout the state. Teams consist of various disciplines and can include geriatricians, medical practitioners, nurses, occupational therapists, physiotherapists and social workers. ACATs assess the physical, medical, psychological and social needs of frail aged people, and help older people and their carers to access services according to their needs.
People who need residential aged care, community aged care packages or respite accommodation must be assessed by one of the ACATs listed below
