Eligibility
People aged 65 years and over (55 years and over for people of Aboriginal & Torres Strait Islander descent) living in country South Australia who have a serious mental disorder which develops in later life or longstanding mental health problems complicated by age-related issues or people who develop dementia with associated challenging behaviours requiring diagnosis, treatment and management of their conditions
Comment
Provides a visiting and/or remote consultation and liaison service to rural and remote regions of South Australia usually on a monthly basis. A consultant psychiatrist visits selected regions every 3 months
Services
- Assessments in clients' usual place of residence
- Telephone consultations for service providers
- Video conferencing assessments by consultant psychiatrists
- Pre-admission and discharge planning for clients entering the unit's Adelaide hospital beds
- Referral to appropriate agencies for long-term management and treatment
- Education services for aged care service providers
- Visiting services are provided to the Hills, Mallee, Southern, Wakefield, Riverland, South East and Northern and Far Western Regions. The first point of contact for clients in these regions is their GP or local community health centre, although direct contact regarding clinical services can be made with a Service staff member covering the region or the Service Co-ordinator regarding service delivery issues.
- A remote consultation service, through the Western Community Team, is provided for service providers in the Mid North and Eyre Regions. As there is no visiting service to these regions, the first point of contact for clients in these regions is their GP or local community health centre. Contact regarding service delivery issues can be made with the Service Co-ordinator
