Eligibility
For Diverse Home Care: frail aged and younger disabled and their carers living in the Mid North region., For a Community Aged Care Package (CACP) or for those with a diagnosed dementia, an Extended Aged Care at Home Dementia (EACH-D) Package: Frail aged individuals with complex needs as assessed by an Aged Care Assessment Team (ACAT) as needing these services and living in the Mid-North region and Northern Areas District Council (formerly Spalding Council), For an Extended Aged Care at Home (EACH) Package: Frail aged individuals with complex needs as assessed by an Aged Care Assessment Team (ACAT) as needing these services and living in the Mid-North region, Clare and Gilbert Valleys, Goyder and Wakefield council areas
Fees
Diverse Home Care: $5 per service capped at $20 per week, Packages of care: Up to 17.5% of the basic pension. Up to 50% of additional income for those on a higher income

- Personal care - assistance with bathing, toileting, dressing and grooming

- Speech pathology
- Home modification - installation of grab rails, handrails and ramps as per assessed need
- Transport - personal transport to medical or other appointments locally
- 2. Community Aged Care Packages (CACP)
- A package of tailored and integrated services such as personal care, assistance with meal preparation, transport, social support, laundry and/or household help. A number of indigenous-specific packages provided
- 3. Extended Aged Care at Home (EACH) Packages
- A package of tailored and integrated services such as personal care, meals, nursing care, laundry, therapy services, provision of equipment, minor home modifications, transport, gardening, shopping, social support and/or household help according to individual need
- 4. Extended Aged Care at Home Dementia (EACH-D) Packages
- A package of tailored and integrated services such as personal care, meals, nursing care, continence management, laundry, therapy services, provision of equipment, minor home modifications, transport, gardening, shopping, social support and/or household help according to individual need. Specialised case management and behaviour therapy planning also provided
