The Home and Community Care (HACC) Program provides a range of basic maintenance and support services to enable older Australians, those with a disability and their carers to live independently. The aim of the program is to support these people to be more independent at home and in the community, thereby enhancing their quality of life and/or preventing their inappropriate admission to long term residential care., HACC is a national Commonwealth & State/Territory cost-shared program which does not provide direct services to the public but funds a wide range of organisations throughout Australia to provide these services to one or more of these target groups. In many cases, the organisation receiving funds contracts out all or part of the services to other agencies., HACC services can be obtained from most Councils, For HACC services in their area, people should contact Commonwealth Carelink, Freecall 1800 052 222
Services
- Services funded through the HACC Program include, but are not limited to
- Allied Health Care
- Nursing care - provided by a Registered or Enrolled nurse
- Personal care - assistance with self-care tasks such as bathing, toileting, dressing, grooming and eating
- Meals and other food services
- Domestic assistance - cleaning, dishwashing, laundry, ironing, shopping, bill paying
- Home maintenance - minor work such as changing light globes, repairing doors and windows, replacing tap washers, lawn mowing, rubbish removal
- Home modification - including installation of rails, ramps, emergency alarms, minor renovations
- Transport
- Respite care - to give carers time away from their caring role
- Counselling, support, information and advocacy
- Veterans and war widows may be eligible for a similar range of support services from the Department of Veterans' Affairs Veterans' Home Care program
