Eligibility
Older people living in the West Torrens Local Government area., People with complex needs such as medical, physical, social and or emotional needs that require support., People approved by the Adelaide Aged Care Assessment Team as requiring this level of support (the approval must be current)., The Elder Choices program is also committed to providing up to 30% of care places to eligible clients from a non-English speaking background., Priority will be given to people most in need.
Services
- Elder Choices aims to provide support and assistance to make it possible for older people who are frail and/or disabled to enjoy quality of life whilst living in their own home.
- Services provided include personal care, meal preparation, podiatry (ad hoc), physiotherapy, transport, equipment/walking aids, cleaning, socialising. lawn mowing, laundry, personal alarm installation/monitoring.
