Eligibility
People 65 years and over and indigenous people 45 years and over who have a serious mental illness or disorder which develops in later life or a longstanding mental health problem complicated by age related health issues who require diagnosis, treatment and management of their psychiatric conditions, or people under 65 years who develop dementia with associated behavioural and psychological symptoms of Dementia (BPSD) who require diagnosis, treatment and management of their psychiatric conditions
Comment
Community Services include consultation, liaison and advice for clients. Assessments are generally conducted in the client's place of residence. Following assessment, a multi-disciplinary team, which can include psychiatrists, nurses, occupational therapists, social workers or psychologists, will monitor the implementation of the recommendations for a short time. The client will be referred back to their GP for ongoing management. However, in a minority of cases, members of the team will provide the case management of services
